Donation Form Donor and Gift Information
To gather details about donors, such as their phone numbers and whether they'd like to give anonymously, add components to your donation forms from the Form designer. From Tools, Donation form designer add or edit a form. Under Form, select and move a component in the Form designer to your form.
Note: To edit default components and those you've added, hover over the section to change and select Click to edit. Address is the only component you can't edit.
To move components that gather information about the gift to a form, select Gift information in the Form designer.
To enable businesses and other organizations to support your online giving initiatives, add Organization gift to your donation forms. When a donor submits a gift on behalf of an organization, the form applies the donation to the organization’s constituent record and the gift appears in Gift management. The form checks whether the person who submits the form and the organization exist and if they share a relationship. When there’s no relationship, the form adds one under Relationships on both records. For more information, see Relationships.
To move components that gather information about the donor to a form, select Donor information in the Form designer.

Note: The Yes/No field is a checkbox. When a donor leaves it unchecked on a form, a "No" response is recorded.
When you add a custom field to a form, you can change the label that displays and select whether a response is required.
Note: When a field's configuration is set to required or only allows one response per record in database view, it can't be changed in the web view.
Tip: To configure and add new custom fields in the database view, from Configuration, select Attributes. After you configure an attribute or custom field, you can add it to donation forms. If a field is set to only allow one per record, any existing data in the field is overwritten. For more information, see Custom fields.
